Parainfluenza viruses are a group of common respiratory viruses that can cause a range of illnesses, including the common cold, croup, bronchitis and pneumonia. What is parainfluenza (HPIV)? Understanding the parainfluenza virus
Parainfluenza virus (Human Parainfluenza Virus, or HPIV) is an enveloped, negative-sense, single-stranded RNA virus belonging to the Paramyxoviridae family (CDC, 2026).
Anyone can catch parainfluenza. The virus is one of the leading causes of upper and lower respiratory tract infections in both children and adults. More than 75 per cent of children over the age of five and roughly 90 per cent of adults have been infected with parainfluenza at some point (Cleveland Clinic, 2022), which means nearly everyone encounters the virus at least once during their lifetime.
The four types of parainfluenza virus
Parainfluenza viruses are divided into four types, each with its own clinical profile and seasonal pattern:

HPIV-1 is the type most commonly associated with croup in children. It also causes other upper and lower respiratory tract infections or cold-like symptoms, and tends to be most active in autumn (CDC, 2026).

HPIV-2 produces symptoms similar to those of HPIV-1 and is likewise more prevalent in autumn, although it is detected less frequently than types 1 and 3 overall (CDC, 2026).

HPIV-3 is more likely to cause bronchiolitis and pneumonia. It usually circulates in spring and early summer but can appear at any time of year (CDC, 2026).

HPIV-4 (including subtypes 4a and 4b) is detected less often. It can cause mild to severe respiratory infections and appears to be most active during autumn and winter (CDC, 2026).
Is parainfluenza contagious?
Yes. Parainfluenza is highly contagious. The virus spreads easily from person to person, mainly through direct contact with respiratory droplets produced when an infected person breathes, coughs or sneezes. HPIV can survive in airborne droplets for more than an hour and on surfaces for several hours (Elboukari and Ashraf, 2023), which means you can also pick it up by touching contaminated objects. Infected individuals are most contagious during the early days of illness.

Symptoms in adults
In adults, parainfluenza typically causes an upper respiratory tract infection with relatively mild symptoms, including:

Runny or blocked nose

However, for older adults and people with weakened immune systems, the infection can be more serious and may progress to bronchiolitis or pneumonia (CDC, 2026).
Symptoms in children
In children, parainfluenza can lead to more severe lower respiratory tract infections. Common symptoms include:
- Ear pain, poor appetite and irritability
- A harsh, barking cough
- Hoarse voice
- Stridor (a high-pitched sound when breathing in)
- Persistent wheezing

Multi-pathogen rapid antigen test kits are available over the counter and can screen for common respiratory pathogens, including influenza, COVID-19, respiratory syncytial virus (RSV) and parainfluenza. Treating parainfluenza infection
There is currently no antiviral medication that specifically targets parainfluenza (Elboukari and Ashraf, 2023). Most infections are mild and can be managed with supportive care at home.
Common approaches include:

Staying well hydrated

Over-the-counter antipyretics and analgesics (such as paracetamol or ibuprofen) to ease fever and discomfort

Cough suppressants, mucolytics or throat lozenges to relieve symptoms
Treating HPIV-1 and HPIV-3 infections
Types 1 and 3 are more likely to cause bronchiolitis and pneumonia, and type 1 is closely linked with croup in children.

Bronchiolitis: Treatment focuses on symptom relief, including antipyretics, adequate fluids and, where necessary, antiviral medication. If breathing difficulties or dehydration develop, hospital admission may be required for oxygen therapy or intravenous fluids (Cleveland Clinic, 2024a).

Pneumonia: Management depends on the cause and severity. It may include antibiotics, antivirals, antifungal medication, oxygen therapy, intravenous fluids or pleural drainage (Cleveland Clinic, 2024b).

Croup: Oral or intramuscular corticosteroids may be required (Elboukari and Ashraf, 2023).
How to prevent parainfluenza infection
There is currently no vaccine against parainfluenza (CDC, 2026). You can reduce your risk through everyday hygiene measures:

Wash your hands regularly with soap and water, especially before touching your face

Avoid close contact with anyone showing respiratory symptoms

Clean and disinfect frequently touched surfaces such as door handles, phones and keyboards regularly, and cover your mouth and nose with a tissue or your elbow when you cough or sneeze

During peak respiratory-infection seasons, try to limit taking young children to crowded indoor spaces

Wear a mask when going to crowd gatherings or packed public transportation

When to see a doctor
Most parainfluenza infections resolve on their own. However, you should see your GP promptly if: your fever lasts more than five days or your temperature exceeds 39°C; you have difficulty breathing or shortness of breath; your child develops a barking cough or stridor; your symptoms are getting worse; or you belong to a higher-risk group (older adults, immunocompromised individuals or those with chronic conditions).
