Why Am I Always Tired? 7 Symptoms of Chronic Fatigue Syndrome

April 28, 2025

Quick Facts on ME/CFS Management

  • Clinical Diagnosis Threshold: Chronic Fatigue Syndrome (CFS) is a clinical diagnosis that requires symptoms to persist for 6 months or more and are not relieved by sleep or rest.

  • The "Exertion Crash": A hallmark of CFS is a significant "crash" or worsening of symptoms following even minor physical, emotional, or mental activity, which can last for days or weeks.

  • Effective Management Strategies: There is no universal cure, so management focuses on symptom relief through Functional Medicine, personalised nutrition, and sleep hygiene.

Feeling constantly tired, accompanied by back pain and other discomforts, is often dismissed as a lack of good sleep. However, it might actually be Chronic Fatigue Syndrome (CFS), a condition that many people unknowingly suffer from. Let’s break it down in detail below!

What Is Chronic Fatigue Disease?

Chronic Fatigue Disease, more commonly referred to as Chronic Fatigue Syndrome (CFS), also called Myalgic Encephalomyelitis (ME), is a serious and long-term medical condition that causes extreme, debilitating fatigue. This fatigue is not relieved by rest or caused by other medical conditions. It often interferes with daily life and can persist for months or even years.

7 Symptoms of Chronic Fatigue Syndrome (CFS)

Chronic Fatigue Syndrome (CFS) is a complex disorder characterised by extreme fatigue that doesn’t improve with rest and cannot be explained by underlying medical conditions. The said symptoms are of moderate to severe intensity for at least half of the time. Here are 7 key symptoms of CFS:

  • Extreme Fatigue: Persistent exhaustion that lasts for 6 months or more, not alleviated by rest, and worsened by physical or mental activity. This is one of the three main symptoms for diagnosis.
  • Unrefreshing Sleep:  Despite sufficient sleep, individuals wake up feeling unrefreshed and tired. This is one of the three main symptoms for diagnosis.
  • Post-Exertional Malaise (PEM):  Worsening of symptoms following physical or mental exertion, which can last for days or even weeks. This is one of the three main symptoms for diagnosis.
  • Cognitive Impairment ("Brain Fog"):  Difficulty concentrating, forgetfulness, and poor memory, often referred to as "brain fog." This is an additional symptom to fulfill diagnosis of CFS.
  • Muscle and Joint Pain:  Unexplained pain in muscles or joints without visible redness, swelling, or inflammation.
  • Frequent Sore Throat or Headaches:  Recurring sore throat and headaches that may vary in severity.
  • Enlarged Lymph Nodes:  Swollen lymph nodes, especially in the neck or underarms, which may be tender to the touch.

Risk Factors for Chronic Fatigue Syndrome

  • Stress: Prolonged stress from work, family, or interpersonal relationships increases the likelihood of CFS.
  • Viral Infections: Some CFS patients have a history of infection with human herpesvirus type 4 or 6 (HHV-4/HHV-6), Epstein-Barr virus (EBV) and human parvovirus B19.
  • Immune System Issues: Many CFS patients show signs of immune system dysfunction.
  • Hormonal Imbalances: Dysregulated  hormone production & interactions from the hypothalamus, pituitary gland, or adrenal glands is often observed in CFS patients.
  • Physical or Emotional Trauma: Experiencing physical injuries, surgeries, or emotional distress may trigger CFS.

Managing Chronic Fatigue Syndrome

There is currently no specific cure for CFS, and treatment focuses on alleviating symptoms. Commonly used approaches include:

1. Medication:

  • Low-dose antidepressants can improve sleep quality and relieve pain.
  • Nonsteroidal anti-inflammatory drugs (NSAIDs) can help with headaches, muscle pain, and fever.

2. Counseling or Psychological Support:

3. Treating Insomnia:

  • Avoiding caffeine or using oxygen therapy may improve insomnia and sleep apnea.

4. Functional Medicine:

  • Functional medicine focuses on identifying potential underlying health issues. This science-based, holistic approach involves personalised nutrition, diet, and exercise plans to restore health and prevent disease.

The "Hong Kong Burnout" vs. Chronic Fatigue Syndrome

Hong Kong is known for its "work hard, play hard" culture, and many residents often dismiss extreme exhaustion as a natural byproduct of the local lifestyle. There are distinct clinical differences between work-related burnout and Chronic Fatigue Syndrome (CFS).

Recognising the Difference

While both conditions share symptoms of lethargy and reduced productivity, their root causes and recovery patterns differ:

  • Burnout: A response to prolonged workplace stress or emotional exhaustion. Burnout symptoms usually improve significantly after a period of rest, a long holiday, or a consistent change in sleep habits.

  • CFS/ME: A systemic medical condition. Unlike burnout, the exhaustion of CFS is not relieved by rest. Taking a holiday or engaging in light exercise can sometimes trigger a "crash" (Post-Exertional Malaise) rather than providing recovery.

The causes of Chronic Fatigue Syndrome can be complex, so it’s important to work with a doctor to create a tailored treatment plan. You can consult a healthcare professional for a functional medicine approach to develop a strategy that works best for you.
